V1.5.25 Fan Logic

Please describe exactly how the cooling fan logic works for firmware version 1.5.25 for the following switch models:

WS-8-150-AC
WS-12-250-AC
WS-12-400-AC
WS-26-400-AC

Here is the syslog entry from one of the switches (!Fan 2 is failing (0 RPM, cooling level 255, board temp 41 C).

I am setting up my Zabbix triggers for failed fan(s) and am getting false triggers, so I need to know your exact logic in order to set up the trigger properly.
