LAG Port State Not Active

What would cause 1 out of 2 ports in a LACP-A group to show "not active." The physical port is enabled and up.

What firmware are you running, the reason I ask is we kinda broke LACP in v1.5.1rc2 inadvertently when fixing something in MSTP but was fixed in v1.5.1rc12 when brought to our attention. I do not think this is your issue BUT if using v1.5.1rcX make sure it is rc12.
You would have to provide a lot more information to even make a guess.
