WS-12-250-DC --> 24v & 3v out of range

This is a unit that I have had brand new in the box on the shelf as a spare. I got it out a few weeks ago to use it and even after upgrading the firmware to the latest, I'm getting an out of range error on the 24v & 3v boards; even with only 2 devices plugged in and neither of which have POE enabled. I checked the MAC and it's saying that it's out of warranty.
Please advise.
